PLACER COUNTY, CA (MPG) – Placer Community Foundation proudly administers a powerful collective of health and human service providers known as the Placer Collaborative Network (PCN).

Over the years, Placer Collaborative Network has served as a catalyst for solving complex community issues through trust, innovation and partnership. Born in a time of rapid growth and shifting systems, Placer Collaborative Network began as a way to unite Placer County’s nonprofit leaders, government departments, healthcare providers and educators around a shared vision: to ensure residents are safe, healthy, housed, in school or employed and on a path towards a prosperous life.

Starting in 1995, Placer Collaborative Network championed prevention over crisis response, using the iconic “waterfall” diagram to illustrate how timely support can stop people from “going over the edge.” With limited resources and a largely rural population, Placer stood out not by numbers, but by pioneering a model of collaboration rooted in shared outcomes and real relationships.

The heart of Placer Collaborative Network became its annual Collaborate Annual Meeting for Placer (CAMP) where stakeholders from every sector gathered in a safe, facilitated environment to reflect, recharge and design new solutions. These gatherings led to the creation of impactful programs like family resource centers, youth resilience initiatives and culturally responsive outreach.

Today, Placer Collaborative Network continues to help weave a strong fabric of trust across organizations. It enables faster data sharing, smarter grant writing and lasting partnerships. With three decades of collaborative practice behind it, the Placer Collaborative Network remains a think tank, a connector and a compass guiding community leaders through evolving challenges – always centered on prevention, shared vision and collective impact.
